Lessons learned from:

American Airlines Flight 191


American Airlines Flight 1420:

Important lessons were learned:

After the accident, “American Airlines revised their checklist procedures, both pilots must confirm that the spoilers are armed, ready for landing”.

The pilots had been suffering from a condition known as 'Get-There-Itis'. The Narrator told the audience that “The pressures of Get-There-Itis sparked a fatal series of mistakes and misjudgments. At the end of a long day, rushing to beat the storms and get the passengers to their destination. The Crew of Flight 1420 made the basic mistake that cost 11 people their lives, they forgot to arm the spoilers”.

The Narrator told the audience that “After the accident, American Airlines revised their checklist procedures, both pilots must confirm that the spoilers are armed, ready for landing”.


The Narrator told the audience, “In October 2001, the National Transportation Safety Board (NTSB) published their report, they concluded that the 2 main causes of the crash were first, the decision to land in a thunderstorm and second, the pilots failure to arm the spoilers”.



Flash Airlines Flight 604:

Important lessons were learned:

Flash Airlines eventually went out of business.

“As a result of the Flash Airlines Flight 604 tragedy, new rules came into place to ensure that in the future, aircraft safety violations will be judged more harshly”.

David Learmount (Safety Editor, Flight Global) “The Flash Airlines crash gave the final political impetus, to a move to create a European blacklist, where if one state banned an airline, then all the other European Union states would automatically ban that airline also”.

All airlines have to give their pilots and cabin crew, ‘Crew Resource Management’ training.


Crew Resource Management

Crew Resource Management is a team work training program that trains pilots to work as a team rather than as individuals. Flash Airlines went out of business because they did not give their pilots Crew Resource Management. All airlines have to provide their pilots and cabin crews with Crew Resource Management training. A number of other businesses and emergency services are using versions of Crew Resource Management.

After the accident, “Airbus issued a bulletin reminding all pilots how to use the rudder”.

“American Airlines modified their training program”.
